The less-discussed version can cost practices just as much: failing to bill for care that is genuinely separate from the original procedure but happens to occur during the global period. An unrelated E/M service may require modifier 24, while staged, related, or unrelated procedures during the postoperative period may require modifiers such as 58, 78, or 79, depending on the circumstances. Documentation must clearly establish why the service is separately reportable rather than part of routine postoperative care.

Modifier issues show up across nearly every category of orthopedic coding, and the reason they’re so persistent is structural: once a wrong modifier pattern gets built into a billing template, every claim that runs through that template repeats the same error.

Reporting both codes separately without checking whether an edit applies, or without the modifier needed to justify separate reporting when it’s genuinely warranted, is one of the more common and avoidable sources of automatic claim reductions. Documentation Gaps: Injections, Imaging Guidance, and Fracture Specificity

A few documentation patterns show up repeatedly across orthopedic denials. Injection procedures are billed with ultrasound guidance codes when the note only supports a standard “blind” injection: the guidance code needs its own explicit documentation, not an assumption based on how the practice usually performs the procedure. Fracture care coding needs enough specificity to reflect the actual fracture type, site, and treatment approach (open versus closed treatment, with or without manipulation) rather than defaulting to a general code that doesn’t fully capture what was done. And operative notes that describe the procedure narratively without clearly identifying the specific anatomical structures addressed make it difficult for a coder or an auditor to confirm the code selected actually matches the work performed.

Medical Necessity and Prior Authorization for High-Cost Procedures

Even a correctly selected CPT code can fail if the diagnosis code attached to it doesn’t clearly support medical necessity for that specific service. This matters most for higher-cost orthopedic procedures like joint replacements, spine surgery, and advanced imaging, which frequently require prior authorization before the procedure happens at all. Confirming both the authorization and the diagnosis-to-procedure linkage before surgery, rather than after the claim is submitted, prevents a denial that has nothing to do with how well the procedure itself was coded.
